.env instructions #138

The install instructions state, "Create a .env file or rename .env.ondemand to .env and modify environment variables." but there already is a .env in the 1.2.0 tag and no .env.ondemand.

The .env file isn't as complicated as for the rails apps, so I just added it.

I've added comments to the file and updated the instructions that it may need to be edited.
