Add warning when shell session gets disconnected #35

Currently the terminal just freezes and the user has to refresh the page to get reconnected. We should pop-up a message saying their connection was terminated and to refresh the page.

Secure shell for Chrome does this:

We probably won't be able to do "Choose another connection" or "Exit" since OOD Shell isn't a Chrome extension. But we can supply a "(R)econnect".
