Include Feedback guidelines within the document #289

Applicable to Security & Governance Checklist (from here).

Review the table of contents to see that the instructions for giving feedback on the document are not included within the document itself.

I believe it would be helpful for readers (especially if they simply started with the PDF - lacking the "project page" context) to have this handy. It may lead to more feedback. And may lead to lower noise for the team explaining how to give feedback.
