adding detailed contributions of all authors #120

We (@patricialg and myself) tried to list all contributions we kept track of. If anyone feels we have missed something please let us know. Our goal is to list the contributions as detailed as possible, which will not only show the important work of everyone but also will help to assign editors/reviewers during the community review.

@tomhull could you please review and add missing contributions you are aware of accordingly?
