Can no longer create new incidents or update existing incidents #222

When someone tries to create or modify an incident, the form tries to load but spins and never loads. You can see the form greyed out in the background. TEOC1

It did work because we created a couple of test incidents but now it's no longer working. TEOC2

I've uploaded a screenshot of the screen as it's trying to load and I've also uploaded a screenshot of the developer tools console that shows the error that it can't find a dropdown option.

@kweenmcse2k6 , thanks for reporting this issue. According to the screenshots shared, it appears that some lists may have been deleted from the TEOC Site. Could you please confirm whether any lists have been deleted? Additionally, please open Teams in your browser, enable the developer tools, then launch the app and provide the console logs and network trace to assist in our troubleshooting process. Master Lists -

I had already checked to see if any lists had been deleted and they hadn't. However, this morning I checked and they did rename the TEOC-RoleAssignments list to another name. Once I changed it back to the original name, it is back up and running.

Thanks for replying. The RoleAssignments list had been renamed by an end user. I’ve named it back to TEOC-RoleAssignments and the app is working as expected.
