DECSCNM should only affect default foreground and background colours #115

DECSCNM reverse video mode currently swaps foreground and background
colours for all character cells, including those with explicit colour
attributes. It should affect the default foreground and background colours
only, as in xterm.

In inverse mode with the cursor colour set to the same as the text colour the 
cursor
remains invisible, even when blinking is on.
