Update permissions table generation

This PR:

• Removes $ref, $value and $count from path prior to permissions lookup
• Adds ms.localizationpriority and ms.topic metadata attributes to included permissions file
• Moves permission table to own file only if it's been updated from Kibali
• Adds the ability to skip update of permissions table by adding <!-- { "blockType": "ignored" } --> before permissions table
• Add permissions boilerplate if missing and we have new data from Kibali
