ActorIdentifier is unique for an identifier and a namespace #27

An identifier should be unique for an identifier and a namespace; otherwise it's not a very good identifier!

So, we'd have

actor_id identifier namespace
US 840 ISO 3166 numeric
CA 124 ISO 3166 numeric
US USA ISO 3166 alpha-3
