I personally have the same issue on 4.3.1. It is a problem with how the links are stored by default in the Classic style menus, and is related to #522 and #532. There is a way to bypass this, as I shared here.
and by the way @molitar , you use a normal not fixed release, take 4.4.169 nightly :

@bonzibudd, Bonzy, you should also check the Versions of the used OShells before you help, could be fixed in a more actually version .. and, you could it add it into the wiki for easy linking and Actualization the Wiki at "known Bugs and behave (and their solutions)" ..

Describe the bug So I pinned an app to the Open-Shell Start Menu open up the regular Windows 10 Start Menu and I now have a duplicate app with a blank white icon in the start menu. Did not have this problem with Classic Shell 4.3.1.
To Reproduce Steps to reproduce the behavior:
Expected behavior Pinning app to Open-Shell start menu should not affect Windows 10 Start Menu

Additional context For now guess I'll go back to Classic Shell it does not seem to have all the problems Open-Shell does.. Open-Shell refuses to see Microsoft Edge icon always shows it blank and now this duplication bug.
