Aciclovir and midazolam examples #61

Includes the code, model and data for midazolam parameter estimation example.

Iterations were double counted in the current code (when printIterationFeedback was TRUE). Fixed.

Merged the changes in the aciclovir-and-midazolam-examples branch into the algorithm-selection branch. Further updates to the example code will happen in the algorithm-selection branch.
