Remove Request Access feature properly #647

Should be removed. Ideal candidate for extra extension if not.

It was removed by 0999fcb6af0d4c7b665479a3a73d52a29d80c73a for commit #270. For this issue, clean up and remove properly. Maybe it will come back as an extension later.
