Add extra provenance fields #2

We need Git Repository URL (with Sha as a fragment) and command to run. For example

repo: git@github.com:OpenAddressesUK/ernest.git revision: ss09393nwrlwj command: bundle && rake import

We could also ask for a Vagrantfile to configure the box environment, but this could probably come later

@pezholio what's the status of this? the README still only has the date & URL fields - is that up to date or should the structure above be reflected in the the README?

I think this is all done, but the README looks like it needs an update, yes.
