Do not include "Show" History checkbox when printing forms from the Participate view #152

Since our Participate forms do not include item history, the option to include history in printed versions of Participate is unhelpful and potentially misleading. The checkbox should be removed from that view and internally defaulted to unchecked.

To who ends up doing this: Follow the code in enketo-express-oc and let me know if you have any issues.

Makes sure to test both Grid and OC theme forms.
