Use canonical URL where possible in the "system" field of a ValueSet #1610

The "system" field in the ValueSet is using OCL's relative URL, not the ValueSet's canonical URL (e.g. "system": "/orgs/CIEL/ValueSet/CIEL/") (see here for example: https://fhir.staging.openconceptlab.org/orgs/MER_SMART_Guidelines/ValueSet/art-medications/?version=v3) - In the example, I would expect CIEL's canonical URL "https://cielterminology.org/" to appear in the system field.

In the long run, we probably want to use canonical URLs by default, but that's a decision we need to make.
