Cleanup/naming of stepup #42

The OpenConext term is "Stepup" (not SSA), rename the remaining ssa occurrences so we do not confuse with two terms for the same thing. Update docs as the PDP rules are not interpreted exclusively anymore by the Stepup-Gateway.
