Add entityID to the list of not used providers #185

This issue is imported from pivotal - Originaly created at Sep 5, 2018 by Bart Geesink

The current list of providers that are not used do not show the entityID, and have empty names sometimes. Adding the entityID ensures that we know what entity it is
