As the Gateway I should log all authentication transactions so that reports can be created #368

This issue is imported from pivotal - Originaly created at Feb 3, 2015 by Daan van Renterghem

The log entry should be made when a user is done authenticating in the gateway, and should contain:

IdentityID
Second Factor ID (if loa > 1)
Second Factor Type (if loa > 1)
Institution
Authenticating IdP
Requesting SP
Authentication Result
Resulting LOA (if succesful)
DateTime

The log-table should be readable by the Middleware so that that can report on it through a back-end API
