Make both the provider-name and provider-address active links to its provider page.
At first I'd thought the entire provider-tile would be a link to the provider page, but considering it'd be frustrating for someone to think they're about to click on the provider's url and then have to wait for the provider page to load before clicking on the url to the provider's website. And so now I think it makes more sense to only have the name and address be active links, as the physical address is represented on a map on the provider page. (Plus, when a short description runs long and gets abbreviated with a "...read more" that entire short description will be a link to the provider page.)
