Remove HazardousRiskIndicator from MainCarriageShipmentStage #166

I cannot see a case where HazardousRiskIndicator on the main carriage shipment stage is different from the one on the consignment level. Therefore I propose to remove it from TransportExecutionPlan and TransportExecutionPlanRequest.
