In the recent changes we removed support for title / subtitle but didn't remove the UX.

We should re-enable support for title / subtitle.

I could support a default title / subtitle (so if nothing is set it will say "A Simple Book" as the title)

For now I'm saying if there is not title, SimpleBook will not generate a title page.
