On https://ttm.lisc-chicago.org/lsna/programs/program_profile.php, there's one place where the instructions next to a form field say:
Dates must be entered in the format MM-DD-YYYY (or use the pop-up calendar).
But no pop-up calendar is visible, even if one clicks in the field.
