Upgrade the OTI indexing script to adapt to change in OPENTREE_API_BASE_URI (in deployment). Also some cleanups and generalizations; and a Makefile for local testing.

This PR must be done in concert with the corresponding germinator PR https://github.com/OpenTreeOfLife/germinator/pull/96
