Remove Badges When Indexing #34

If a badge is removed from an issuer provided endpoint list of badges, it should also be removed from the directory.

Add endpoint to meta data. When re-indexing - remove indexed items based on endpoint and then re-index.

It now happens as part of the indexing, using the deleteByEndpoint method
