Feature: Mentor form modifications #1044

Make the mentor form easier and user friendly

Why is this feature being added?

Couple of issues are occuring.

  1. People input the wrong email or information
  2. People don't know to join slack and we can't get in touch with them.

What should your feature do?

  1. Auto populate user email from their session information.
  2. Add text to the form like please join slack so you can talk to your mentor with a link to any of the following: slack.com operation-code.slack.com intro to slack page
