Add code school request form link to Code Schools page #986

Detail required: Where on the page do we want to display the link?

I am happy to tackle this, as soon as I get some guidance about where to put the link

A simple one-liner with a link, immediately below the "We encourage you to check out the schools below, do your research, and ask fellow techies in our Slack Community." would work for me.

Something like "Code schools - if you'd like your school to be listed here, please fill out our request form."

Thoughts @dmarchante @kylemh ?

Ok, sounds good. I will submit a PR shortly, and happy to incorporate any feedback on styling once folks see it.
