Added code school request form link to Code Schools page #989

Description of changes

Added a line to the Code Schools page with a link to the request form so schools can submit their information. The line reads "Code Schools: if you'd like your school to be listed here, please fill out our request form."

Issue Resolved

Fixes #986 "Add code school request form link to Code Schools page"

Done! Not sure if that's the proper usage of the component, but let me know if there's something that needs fixing and I'll get to it asap. Thanks for the feedback.
