Does it happen with only MalisisDoors installed ?
I can't tell as this (see Image) is the only constellation it did crash until now. I can't reproduce this constellation without any other mods, as the world would be completley empty then and the same Situation can't be made again.
But i'll try tomorrow.
One Player thankfully tested this for me, as i am not able to remove optifine (Need it for Resolution scaling or i can't Play on 4K Monitor as even my graphics Card can't handle minecraft in 4K at this Point) and still had the Crash with it. Without it was gone, my Player said. It seems like Optifine is unfortunatley once again the cause for such a bug, we had one the last days, too. I reported over there.
I could not replicate, even with Optifine installed.
Eh. Okay. But you see i got a crash log that does proof it did happen, do you? :D I also reported it to optifine, as it only occurs - reproducable - with it.
So yeah, i still can reproduce this in the exact circumstances seen on the screenshot and still get the same error.
The crash is in the MalisisDoors classes when rendering a block entity.

Does not seem to be related to OptiFine.
Fixed in MalisisDoors 1.12.2-7.2.0.
See in this Picture, for every single of those trapdoors placed, the following Crash occured: https://ibb.co/fODSgG
