When anonymizing, create separate non-anonymized metadata file #15

When anonymizing a file for distribution, it would be useful for somebody to know the true data for that file.

We now create a file in the output directory called <input file base name>-anonymized-storage.txt that contains MRN, Patient Name, Unit, and Bed, if the data was stripped from the input file.

This only works with the --anon command line argument during conversion. It doesn't (yet) work for anonymizing and existing file a la #28
