It's not really working with Colorful Hearts on 1.20.1 fabric

is it a Colorful Hearts issue? What's occurring?

Colorful Hearts was working fine. Your mod wasn't working. I've set myself on fire and just had the normal hearts behaviour

It looks like Colorful Hearts rewrites the heart rendering system, overriding the changes from my mod. Don't know of any good fixes on my end of things currently
