In circuit-level noise definition, split initialisation error probability into 'initialisation' and 'reset'? #33

Based on IBM Heavy Hex lecture by Ben Yoder. They consider 'initialisation' to be a reset that happens just once, right at the start of the circuit. Then 'reset' is any later reset in the circuit, e.g. on an ancilla used for syndrome extraction.
