IB subsystems use common ID scheme #43

For convenience, order, hist data and other subsystems each use independent
ID schemes.  Errors only give the passed-in ID making it
difficult/impossible to associate the id with the request in the event of
conflicts.

Need to unify so that an ID can be uniquely associated to a request.
