Convert check failed cases to old format #67

We didn't convert the cases which failed the first step of quality check. These cases are listed in json_check_failed in quality_check.json.

For example, 144456, 245376 and 212516 are the cases which failed the first step of QC. We didn't convert these cases to old format.

Now, we need to convert them to old format even if their diagnosis are from microarray of FISH.
