Update sidebar link names and title attributes

This issue covers suggested changes to the links in the sidebar.

Rename link names

My first suggestion is to rename some links in the sidebar so that their purpose is more obvious:

In the sidebar, rename the 'Wiki page' link to 'Wikipedia page'.

In the Data links drop-down menu, rename the following links:

PHI-Canto issue tracker	Curation issue tracker
PHI5 UI issue tracker	PHI-base 5 website issue tracker

Link title attributes

The table below shows the suggested text for the title attributes for sidebar link. The title attribute is the text that is shown when the link is hovered over. For example:

Some notes on the table:

• The link names shown below assume that the new names given above are being used.
• I've excluded the links that are being considered for removal (see issues #40 and #41).
• Link names that are not in bold are links contained in drop-down menus.
• There are no titles for the links in the 'External resources' drop-down menu, since I don't think they're necessary.

Diseases	Download a table of diseases used in PHI-base.
