Rename HR-induced host lesion terms #377

The term names of some host lesion terms are pretty confusing at the moment. Consider the following:

'decreased extent of host-defense induced lesion by host hypersensitive response' (PHIPO:0001204)

It seems redundant to include 'host-defense' when the term name already includes 'host hypersensitive response' (a type of defense response).

I'd recommend renaming terms like these to something like the following:

'decreased extent of lesions induced by host hypersensitive response'

@jseager7 Thanks for the suggestion. I think I need to review this whole branch before making changes to individual terms see #306. I agree the terms need a tidy up.
