Create xwalk table with: KCMASTER_ID # from IDH id_apde # created by code, when Mcare is linked, will have more distinct values than KCMASTER_ID id_mcaid # from IDH id_mcare # NULL place holder until can link phousing_id # from IDH, 64 character hash
