The ampersand in the 'Invertebrate - Dragonflies & Damselflies' entry in the lu_taxagrp table causes the tool to break on the webserver. It runs fine on desktop for some reason (we think). Changed the entry in the sqlite db to 'Invertebrate - Dragonflies and Damselflies' and it fixed the problem.
I made this change on the master copy of the sqlite db, but we should look for other potential instances that can cause this error.
