install.php hard codes path to extension_custom.conf #18

install.php writes an include statement to extension_custom.conf and assumes that file is located in /etc/asterisk/ Install will fail if that folder is not correct.

The asterisk conf directory should be available from an environment variable.

version 1.3.5 no longer edits custom.conf files, dialplan is written "properly" within FreePBX
