Charge above/below charge #202

What steps will reproduce the problem?

Not currently supported - simplest is to put two charges in pale, but how to 
determine sizes? Should both have same bounding box size, e.g. crown above 
anchor?

charge "with" charge is like charge "below" charge (see example in issue 183)

I am making this into a new type of arrangement. Whichever charge is mentioned 
first will get the largest bounding box,
e.g. A over B is:
   A
   b

A under B is:
   b
   A
