Add human readable label to images #141

When there are multiple to select, good to have a label for them to make it easier to select which one, especially when the filename is opaque. Integrate with multiple image upload, and use in prose mirror #118

As part of #156, the Series will allow uploading up to 2 images. The first will have purpose=profile, and the second purpose=thumbnail, to tell them apart. (Though this still displays filenames in the prosemirror dropdown).

The Story still only allows one image, which has a blank purpose.
