Add 'extra_details' step to create

• Base.pm - Add handling to insert and extra_details step into the create process
• intra-includes/create.inc - Add the form to handle extra_details step

This still needs some work, but I wanted to get a review and some help on it. We're working up to the point of the extra_details step where the form displays perfectly but the POST doesn't seem to do anything and I'm not entirely sure why... we just get redirected back to the illview page but the request isn't created.

I'm sure I'm missing something obvious here.
