Export already implemented functionality of distributed MPI computing to FastIce.

[x] Create exchangers for communication hiding
[x] Add distributed cartesian grid with methods to return the local part of the grid
[x] Export split_ndrange function
[x] Add optional index offset to kernel computation
[x] Change the hide_comm function to notify exchangers instead of spawning new tasks

In addition:

[x] Add cooperative_test! to MPI.jl (https://github.com/JuliaParallel/MPI.jl/issues/762)
