Some UI updates #269

change the header a bit (color + font)
simplify the upload dialog: merge the 2 upload types, remove some fields and add an access dropdown (see below)
store email + access type locally using localStorage

Hey we should take the opportunity to set the dropdown default option to "public", since we are providing a public service their logs could help us with research
