This pull request introduces the following changes:

Add per-instance configurable TCP timeout suggested earlier (see comments on #48)
Bump version to v4.3.0
Drop Python 2.7 support
Clean up and generate documentation pages
Add tests for Python 3.9

Merging this today to mark the end of support for Python 2.7; all future pull requests will not be required to pass Python 2.7 tests.

The new version will be released in PyPI after PagerDuty Summit concludes.
