All sources of damage conversion are additive. The in-game tooltip can never be trusted to be 100% correct as there are so many circumstances it does not cater for. You are most likely dying due to having Frenzy in your manaforged setup

Holy shit this has been bugging me all day and the conversion was all I could think of and it seemed to be supported by the tooltip!!!
I will test a physical reflect map without frenzy

What is the value from the calculation in-game?
I have the following allocated in game and in PoB:
My in game tooltip shows I still have a portion of damage being dealt as physical (it is enough to kill me in physical reflect maps). PoB calculations show I have over 100% physical converted to elemental and deal no elemental damage.
I believe the conversion from tree/gear is additive, but the conversion from the gem itself is multiplicative - therefore I would have 75% phys to cold, 15% phys to random element, 10% phys based on the above example.
What is the value from the calculation in Path of Building?
Path of Building is calculating my physical conversion as over 100% when the in game tooltip still shows physical damage remaining.
