Add 'First item is All' option #135

I'd like to see an alternative to the current firstItemChecksAll.

It would work in a similar way except that if the first item is checked, all 
other items are unchecked.

So then when a form was posted back, only the value of the first item would be 
in the postback data, rather than every value.

Version 1.4 supports a firstItemsChecksAll option of 'exclusive', which 
implements this functionality.
