PayU payment option not showing Billing address or T&C #3

On Checkout, under PayU option, 1) there is no billing address fields 2) no checkbox to "use shipping address as billing address" and 3) no "Agree to Terms and Conditions" link (screenshot of what EFT looks like attached)
