Add a table of contents to the readme (/README.MD).
Context
Currently I have to scroll through the entire thing to find something. Let's add a heirarchical list-style ToC with links to each heading.
The markdown pattern for links is [Link Text](URL), e.g. [Google](https://www.google.com). The "URL" for internal markdown headings is ./FileName.md#Heading_Text, or just #Heading_Text.
There might be an automated way to do this? Take a look.
