Update Readme.md to add porting information #4

Updated markdown formatting for headings and code sections. Added detailed porting information. Still needs work. Needs additional review and links populated.

@MarceloSalazar @maclobdell Thanks, I've updated the README based on the latest version of the example app (and thus could delete 80 lines from the document :-)).

@janjongboom looks good. FYI I've tweaked minor things and added links.

I think the only thing missing is the 'Update the LWM2M objects' section. Do you think you could write something about this? or should we remove it?
