Index of Problems #12

Can we auto-include an index of problems in the Chapters in an intro-page. These should not be expandable, but provide links to the actual problems in subsections.

@davidfarmer : Is this an issue to open with the full MBX project as a feature request? Or is this something that is already built-in somewhere and just not enabled? ... it certainly is something we'd want picked up in a script...

After discussion on the PBX list-serve.. I think I've got an answer to this one (that I can automate, but not needed in the script).
