Individual numbering sequences for: Examples Exercises/ Problems Theorems and Definitions

Theorems and Definitions Numbered for Chapters, not with exercises or problems.

@davidfarmer -- this is definitely a scriptable issue... if it's possible to have uniquely numbered things... not exactly sure how that's worked out in MBX.

Marking this as resolved. I was reading more of the MBX author guide and apparently it was an intentional choice NOT to number these things separately... which means I'm not going to fight it.
